HOW DO I GET FREE PASS?
It’s nothing complicated, on the right side of the page you select the club you want to visit and fill in your contact information. We will immediately send you the necessary information for your visit by email. If you don’t receive your email, please check your advertising or spam folders.
CONDITIONS OF FIRST ENTRY FREE
- Applies only to new customers who have not visited any Form Factory club yet.
- Applies to persons over 15 years of age. Persons aged 15 – 18 years only with the consent of a legal representative.
- There is only one free entry per person.
- Registration is only possible with a Czech or Slovak phone number.
- One-time free entry is valid for 7 calendar days from registration, otherwise it expires.
- Before entering the club, you will be asked to verify your identity with a valid document.
- You consent that Form Factory may contact you (by email, SMS, telephone or other means) for
